San Rafael Industrial Accident Fatality Identified

The decedent has been positively identified as:

Shawn Frederick MOORE, a 39-year-old male from Woodland, CA.

On Monday, December 10, 2018, at approximately 2:45 pm, the Marin County Sheriff’s Office Communications Center received a 9-1-1 call reporting a workplace accident on the Marin Islands National Wildlife Refuge located off the shores of the San Rafael Bay, San Rafael.  Reportedly construction crews had been working on the island to deconstruct a preexisting rock-cement structure.  During demolition, a wall of the structure collapsed resulting in a construction worker being trapped beneath the heavy debris.   As a result of the collapse, the subject suffered extensive crushing-traumatic injuries and immediately had become unconscious, unresponsive and lifeless.  The subject required extensive extrication, which was provided by First Responders, who arrived on the scene a short time after being dispatched to the scene.  Despite the valiant efforts provided by multiple First Responders, the subject passed prior to being freed from the heavy debris atop of him. 

The decedent has been positively identified as:

Shawn Frederick MOORE, a 39-year-old male from Woodland, CA.

An investigation into the cause, mode, manner and circumstances of death is currently underway by the Marin County Sheriff’s Office, Coroner Division and the San Rafael Police Department.  Due to the incident and death occurring on Federal lands, the United States Department of Labor, Occupational Safety and Health Administration is assuming jurisdiction and investigating this incident.   A forensic post mortem examination with toxicology testing has been scheduled for today.   The cause and manner of death will be pending the conclusions of the aforementioned investigation, examination and testing results.  No further information will be released at this time, as this is an active and ongoing investigation.
