Corte Madera Motor Vehicle Collision - Fatality Information

The decedent has been positively identified as:

Samuel Lee OUELLETTE, a 42-year-old male resident of San Anselmo, CA

On Thursday morning, June 18, 2020 at approximately 05:42 A.M., a solo vehicle collision was reported to 9-1-1 having occurred in the 1100 block of Meadowsweet Drive, Corte Madera.  Reportedly a commercial refuse truck, which was operated and solely occupied by the subject was traversing down a steep narrow residential driveway and for unknown reasons, which remain under investigation, left the roadway and rolled over onto its side.  Unfortunately during the incident, it appeared the subject was ejected from the moving truck and was struck by the vehicle becoming entrapped beneath it. Emergency Services were immediately dispatched to the scene, and arrived shortly after discovering the subject unconscious and unresponsive having sustained mortal traumatic injuries. Sadly, his death was pronounced on the scene without resuscitative aid provided.

A forensic post mortem examination and toxicology testing have scheduled for later this week.  The cause and manner of death will be pending the conclusion of the investigation being completed by the Central Marin Police Authority, California Division of Occupational Safety and Health, and the Marin County Sheriff’s Office - Coroner Division.  No further information will be released by the Coroner Division at this time, as this is an active and ongoing investigation.  Any further questions regarding the incident should be directed to the Central Marin Police Authority and California Division of Occupational Safety and Health.
